Filing of Personal Data Sheet Extended Until June 15 – CSC
The Civil Service Commission (CSC) extended the filing of the Personal Data Sheet (PDS) until 15 June 2017. This applies to those who have not yet submitted their updated PDS. Those who have already submitted their updated PDS to their agency’s human resource management office (HRMO) in compliance with CSC Memorandum Circular No. 11, series of 2017 need not re-submit.
Previously set on April 30, the deadline was moved to give government officials and employees more time to fill out the form, fulfill the requirements, and submit to their respective agency’s human resource management officers (HRMOs). The CSC has also clarified certain guidelines through Memorandum Circular No. 16, series of 2017, as follows:
There is no need to put a space in between letters in writing or typing one’s name.
In Part III, under item 26, indicate “elementary” in the third column if graduated in elementary level.
Work Experience section (Part V) should be accomplished and submitted together with the PDS if applying for a vacant position in government. If the PDS is being updated only in compliance to MC 11, s. 2017 it is not required to fill out that section.
The type of learning and development intervention (Part VII) should be identified as either managerial or supervisory, technical, or foundation. For the definitions of each type, please refer to MC No. 16, s. 2017.
The photo to be attached should be passport size (4.5 cm x 3.5 cm). The photo may or may not have a name tag and signature.
The PDS should be subscribed and sworn to before the highest ranking HRMO of the agency, any authorized official/officer in the agency, or any officer authorized to administer oath or a notary public. Heads of agencies may delegate such authority through an Office Order.
The PDS should be submitted to the agency HRMO by June 15. By July 31, the agency HRMO should have already submitted copies of the PDS to the CSC Field Office concerned.
